Warranty Policy

Capitalized terms not otherwise defined in this Warranty Policy have the meanings set forth in the Standard Terms and Conditions to which this Warranty Policy is attached. This Warranty Policy may be modified at any time, for any reason, and at our sole discretion. Revisions to this Warranty Policy will be effective as of the date that we provide you with notice of such change in accordance with Section 16 of the Standard Terms and Conditions to which this Warranty Policy is attached.

Device Warranty:

We warrant to the original purchaser of the applicable Device (“Purchaser”) that the Devices will, under normal use and in accordance with all Documentation, operate in accordance with the applicable scope of coverage set forth below (the “Standard Device Warranty”).

• The Device: Devices will be free from material and or manufacturing defects until the earlier of (1) the date that the Device is used for 3,000,000 waves and (2) the date that is twenty four (24) months following the date of original shipment of the Device.

Consumables Warranty:

We warrant to the Purchaser that Consumables will, under normal use and in accordance with all Documentation, operate in accordance with the applicable scope of coverage set forth below (the “Standard Consumables Warranty”).

• Electrodes: Electrodes will be free from material and or manufacturing defects until the earlier of (1) the date that the electrode is used for 30,000 waves and (2) the date that is twenty four (24) months following the date of original shipment of the applicable Device.
• Cables: Headpiece cables will be free from material and or manufacturing defects until the earlier of (1) the date that the cables are used for 600,000 waves and (2) the date that is twenty four (24) months following the date of original shipment of the applicable Device.
• Applicator Heads: Applicator heads will be free from material and or manufacturing defects until the earlier of (1) the date that the applicator heads is used for 300,000 waves and (2) the date that is twenty four (24) months following the date of original shipment of the applicable Device.

Extended Warranty:

You may purchase additional, extended warranty coverage for Devices as follows (“Extended Warranty” and together with the Standard Device Warranty and the Standard Consumables Warranty, the “Limited Warranties”):

• Extended coverage will either be a “Gold Extended Warranty”.
• Gold Extended Warranty:
  • Cost: $300 per month or $3,000 per year.
  • Contract Term: One-year contracts renewable annually.
  • Coverage:
    • Full Unit Retractions: $500 deductible per claim
    • Top Unit Retractions: $250 deductible per claim
  • Waiting Period:
    • A three-month waiting period applies if the warranty is purchased more than two years after the initial Device purchase.
  • Exclusions:
    • Generator
    • Electrodes
      
    • Applicator heads
    • Handpiece cables
